Lead Screening:
New York State Public Health Law 1370-d states:
Prior to or within 3 months of initial enrollment, schools are requested to
obtain from the child’s parent or guardian, proof that the child has had a blood lead test. This is for any child under the age of 6.
 
Tdap immunizations requirements for the 2026-2027 school year.  All students 10 years or older are required to have their Tdap prior to entering 6th grade. 
 
Meningitis immunization requirements for the 2026-2027 school year, students who will be entering 7th and 12th grade in the fall will be required to provide proof of having received the meningitis shot.  
 
Written proof for both immunizations will need to be submitted prior to September 1, 2026, otherwise your child will not be permitted to attend school after 14 days of noncompliance.  Letters will be sent to parents if your child was in need of either shot.  Please contact Mrs. Brown or Mrs. Hannon if you have any questions.

Health Examination

Requirements for School Attendance                     

Per New York State law, a health examination is required for all students entering the school district for the first time and when entering Pre-K or K, 1st, 3rd, 5th, 7th, 9th , and 11th grade.

The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner and on the approved NYSED Student Health Examination Form for School.

 

  •       A copy of the health examination must be provided to the school within 30 days from when your child first starts at the school, and when your child starts K, 1st, 3rd, 5th, 7th, 9th, & 11th If this is not provided, a health exam will be done during the school year by the school medical director.In preparing for the health exam, students are screened for vision, hearing and scoliosis.  If your child fails their vision and/or hearing screening, you will receive a separate referral in writing.
  •       If your child has an appointment for an exam during this school year that is after the first 30 days of school, please notify the Health Office of the date.

           We suggest you make copies of the completed forms for your own records before sending them to the school health office. Forms may also be faxed to 315-353-2480.
