IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Under the Paperwork Burden Reduction Act, beginning with the 2026 calendar year, employers are no longer required to automatically mail or provide forms 1095-B and 1095-C to all employees.

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 tax year is available upon request. (NOTE: there is no information on the 1095-C form you need to file your tax return for 2025. All information needed to file your tax return is on your W-2 form.)

To request a copy of your form, or if you have any questions, please contact Lisa Mitras in the District Office by email at [email protected] or extension 38734.

Forms will be provided within 30 days of receiving your request. This notice will remain available through October 15, 2026.

2026 NYS Minimum Wage Poster

Retirees who reach the age of 65 are eligible to be reimbursement for their Medicare Part B Premiums. Reimbursements are mailed quarterly in September, December, March and June. Please contact Kathy O'Connell in the Business Office with demographic changes.
